Rental Application and Process
Important: In order to be shown a home, you must turn in a completed application.
NeighborWorks Mountain Country Home Solutions owns and manages several rental properties, some specifically for low-to-moderate income families. Rents range from $750 to $1,100 for a 3 bedroom unit and $875 to $1,200 for a 4 bedroom unit. All security deposits are equal to one month’s rent. When available, we have units in Provo and Spanish Fork.
HOW TO APPLY
- Obtain a NeighborWorks Mountain Country Home Solutions Application form from our office at 39 W 300 N, Provo, UT 84601 or click the button below to download and print the application.
- Submit a completed application with authorizations for the release of information. Upon receipt of a completed application, appointments to see available units will be arranged.
- Documents required with the application form:
- Social security card for each household member
- Birth certificate for each household member
- Photo ID for anyone 18 and older who will be living in the unit
- Bank or credit union statements that have the institution name, member name and full account number printed on the statement
- Income verification – 3 months worth (paycheck stubs, social security, child support etc.)

WHAT TO EXPECT
- If we have a unit available for your family, we will contact you for a screening. We will conduct a home visit to provide us with important information on how well you take care of your present housing unit.
- We will also conduct a criminal background check, run a credit report, contact your employer for income information and obtain reference information from your landlord and/or other references you provide.
- You will then be notified of the acceptance, denial or of any additional information we may need.
Please be aware that acceptance into our units is based on several factors such as vacancy rates, availability, unit size, ability to pay, income level, family size, background check, etc.
